to COVID-19 Precautions and Procedures
Please Read
We hope you are all keeping well and safe. We wanted to give you an update on some precautions the office
is taking to ensure the continued safety for the staff, patients and providers during the COVID-19 health
crisis.
Per recommendations by the
California Psychological Association, during this time, the office will require clients returning to treatment
for face to face sessions to sign an "INFORMED
CONSENT FOR IN-PERSON SERVICES
DURING COVID-19 PUBLIC HEALTH
CRISIS”.
This form will be available for clients to fill out online (see below). In addition we will have
paper copies for clients to sign as well.
The consent is intended to highlight our expectations of the clients and conversely, outline
the safety provisions the patients can expect from the office.
Among other precautions, the
primary change is, that we have implemented a mandatory face mask/face protection rule for all clients
entering the common office areas. Staff will also be abiding by these same
rules.
As before, the office staff continues to work on a reduced hours schedule. We appreciate
your continued understanding as we navigate through these uncertain times. Please feel free to contact the office with any questions or

Your Responsibility to Minimize Your
Exposure To obtain services in person,
you agree to take certain precautions that will help keep everyone safer from exposure, sickness, and possible
death. By signing this document you certify that you agree to the following actions:
1.You agree to keep your in-person appointment only if you are
symptom-free.
2. You agree to take your temperature before coming to each appointment. If it is elevated (100 Fahrenheit or
more), or if you have other symptoms of the corona virus, you agree to cancel the appointment. If you cancel for
this reason, there will not be a cancellation fee.
3. You agree to wait in your car or outside (or in a safer waiting area) until no earlier than 5 minutes before
your appointment time.
4. You agree to wash your hands or use an alcohol-based hand sanitizer when you enter the building.
5. You agree to adhere to the safe distancing precautions in the waiting room and testing/therapy room. For
example, you won’t move chairs or sit where we have signs asking you not to sit.
6. You agree to wear a mask in all areas of the office.
7. You agree to keep a distance of 6 feet and there will be no physical contact (e.g. no shaking hands) with your
provider or office staff.
8. You agree to try not to touch your face or eyes with your hands. If you do, you will try to immediately wash or
sanitize your hands.
9. If you are bringing your child, you agree to make sure that your child follows all of these sanitation and
distancing protocols.
10. You agree to take steps between appointments to minimize your exposure to COVID-19.
11. If you have a job that exposes you to other people who are infected, you agree to immediately let your
provider and office staff know.
12. If your commute or other responsibilities or activities put you in close (less than 6 feet) contact with
others (beyond your family), you agree to let your provider and staff know.
13. If a resident of your home tests positive for the infection, you agree to immediately let your provider
and office staff know and you will (begin) resume treatment via telehealth (or other arrangements).

Our Responsibility to Minimize Your
Exposure
These are the office precautions intended to
further enhance the safety of staff, clients and providers:
1. Office seating in the common areas has been arranged for
appropriate physical distancing.
2. Staff and Providers will wear masks.
3. Staff will maintain safe distancing.
4. Restroom soap dispensers are maintained and everyone is encouraged to wash their hands.
5. Hand sanitizer that contains at least 60% alcohol is available.
6. We ask clients to wait in their cars or outside until no earlier than 5 minutes before their
appointment.
7. Credit card pads, pens and other areas that are commonly touched are sanitized after each
use.
8. Physical contact is endangering (e.g. do not shake hands or
hug).
9. Tissues and trash bins are easily accessed. Trash is disposed of on a frequent basis.
10. Common areas are thoroughly disinfected several times a day.
