Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your
coverage carefully by asking the following questions:
Do I have mental health insurance benefits?
Do I have a deductible and, if so, has it been met? Does the deductible apply to out-patient mental health
How many sessions per year does my health insurance cover?
What is my co-payment or co-insurance owed per therapy session?
Is authorization required from my primary care physician or my insurance company to use my mental health
Do I have employee assistance plan (EAP) benefits either under my insurance plan or separately made
available by my employer?
When you contact us, we also will verify your benefits and obtain the details outlining your financial
responsibility for our services.

Reduced fee services are available on a limited basis.
Cash, check, Discover, Master Card and Visa are accepted for payment. PayPal is available
through our website (Discover, Visa, Master Card, e-Check and American Express accepted for
payment) - here.
When you schedule an appointment with a provider, that time is reserved for you. We do not "double book" or
"squeeze" sessions into the same time slot. Insurance companies do not accept billing of "no show" or late
cancelled sessions. Therefore, if you do not show up for your scheduled appointment, and you have not
notified us at least 24 hours in advance, you may be required to pay a missed session fee. Additionally, each
therapist is free to set their own policy regarding advanced notice times and late cancellation/missed fees.
Associated Therapists, Inc's professional staff are contracted providers in a wide variety of mental and
behavioral health plans. This list is not complete. If you do not see your insurance plan listed below, please call
our office to inquiry about a specific carrier.

For City of Long Beach Employees:
Associated Therapists is the preferred mental health provider for the City of Long Beach Blue
Cross insurance plans.
Accessing services is easy and confidential. The individual calls the office for an appointment. He or she may be
referred by a Primary Care Physician or EAP counselor, however this step is not required. Any person
enrolled in the Blue Cross plans can self refer for Mental Health or Chemical Dependency services to Associated
Initial and continuing visit approval is not needed. Associated Therapists providers have the authority
to approve care within our own practice, thereby assuring confidentiality and faster initiation and continuation of
Associated Therapists has the authority to flex annual and lifetime benefits to maximize the amount and level of
treatment you can receive in any benefit year. Lifetime benefit changes requires the approval by the Blue Cross
