Welcome to Associated Therapists. 

Navigating through the complexities of insurance and managed care authorizations can be confusing to both clients and mental health providers. These required procedures can appear to be a "maze" and pose a significant obstacle in the process of accessing mental health care.

Associated Therapists' mission is to act as a facilitator and to assist in managing the administrative steps that are required in order to see a mental health provider.

In that endeavor, we have created several "tools" to make the process easier and more understandable.

The first step in the process of becoming a client of our providers is to contact our office.

Administrative information will be gathered by an administrative assistant.
1. You will be asked for information about the prospective client.
2. Insurance information will be gathered to assist in verifying your benefits.
3. A very brief description of the problem(s) or services that you are seeking will be
   requested.  This information will be used to match you with providers that are
   most appropriate for your issues.

You will then be contacted by a senior administrative staff member and at that time you will be referred to providers that are the most appropriate for your needs. Location, provider membership in your specific insurance company panel, the specialty and experience of the provider with your issues will be considered.

If your are seeking services for a medication evaluation (Psychiatrists or Nurse Practitioner) an appointment will be scheduled at that time.

If your are seeking services from a therapist for "talk therapy" you will be given the contact information for several therapists so that you can discuss your needs with that provider and make an appointment for your first visit. Please visit the Professional Staff page on this site to see the various providers resumes and locations of service.

After an appointment has been scheduled with any of our providers you may either:

1. Fill out your administrative information on line (Recommended). Click Here.
2. Or print a paper copy of the administrative forms to fill out by hand. Click Here.
3. Or come to your appointment 15 minutes early and fill out the forms in the office.

Our administrative staff is available to help you with questions or concerns that you may have throughout the time you are a client at Associated Therapists. Our staff is available between 9:30 AM to 6:00 PM, Monday through Friday.
